The Senior Care Support Package is designed to give you the peace of mind in knowing that your loved one(s) are well taken care of and being assisted by professionals. We know that you don’t always have the time to give the ones you care about the proper care while taking care of your own family as well. That’s why HomeCare on Wheels offer this basic Home Care and Personal Care support package, so you can be sure that your loved one is getting the care, attention and assistance they deserve. It is important that our clients, and their family members, feel safe, secure and confident knowing we

will be there when needed and ready to help. It is also essential that they come to know the consistency and genuine caring with which we approach health care. We realize one way to make sure of this, and that is to do a good job everyday on behalf of our clients.

The Senior Care Support Package includes:

  • Initial Assessment with our Health Care Coordinator
  • Regular Follow-ups with the Health Care Coordinator regarding the care & services being received
  • Regular Communication with the Family Spokesperson
  • Home Care Services*
  • Personal Care Services*
  • 24 Hour Telephone Support
  • Health Care System Navigation Support*
  • Consult with other health care professionals as necessary

At any time you can adjust the services in your package by contacting our Home Support Team

 

In addition, to these regular HomeCare Support Services, our Home Support Team will, at no additional cost, offer the following:

  • Regularly review and supervise the Home Care Support Services you're receiving, and adjust these services as required,
  • Remain available to you 24 hours a day, 7 days a week, to answer any questions, or solve any issues that you may have regarding your care
  • With your consent, consult with other health care professionals as necessary
